Home Insemination Waiver
This form is required only for home inseminations performed by the patient. We do recommend using a healthcare professional for all inseminations. However, in the event you are not using the help of a medical provider, this form needs to be signed.
Clinical Release Form
This form is required only for vials delivered to your home or picked up at our office. Please have your physician or medical provider sign and return this form.
Recipient Agreement
This form is required for all recipients of donor sperm. If you are a single patient (without a partner or spouse), please leave the “spouse/partner” spaces empty.
Open ID Birth Reporting
This form is not mandatory. However, participation is highly encouraged. This report enables your child to receive more information about their donor and to set-up a meeting with them, after the age of 18.
Pregnancy Report
This form is not mandatory. However, we greatly appreciate those who complete a pregnancy report. This report is used solely for analytics by our team to ensure we are exceeding quality expectations.
Each donor has been tested for 714 recessive genetic conditions, and each has their own unique set of results. As such, each donor has their own specific genetic liability release document that must be signed prior to the shipment and use of that donors sperm. This document can be found on it’s respective donors profile page under the “Donor Downloads” section.
